Below is an example of a form used in a Catholic Church for a baptism.

This form is a testimony for individuals who wish to serve as godparents in the Catholic Church for a person being baptized.

 

The form includes questions about the individual's Catholic faith, including whether they have received Confirmation and Holy Eucharist, and whether they are free to receive Holy Communion. For those who are married, the form also asks about the circumstances of their marriage. For those who are unmarried, the form asks whether they are in a romantic relationship or living with a partner. The individual must sign the form in the presence of a Catholic priest, deacon, or pastoral minister, attesting to the truthfulness of their responses. The form is then signed and dated by the Church representative and sealed.
